A FULL TREATMENT FEE WILL BE CHARGED IF LESS THAN 4 HOURS NOTICE OF CANCELLATION IS GIVEN.
A FULL TREATMENT FEE WILL BE CHARGED FOR FAILURE TO ATTEND WITHOUT NOTICE.
I understand that my contract for physiotherapy treatment is with Melanie Betts and that I am responsible for all payments. This applies to Private Medical Insurance also should they not pay.
Payment is requested following each treatment; unless claiming through Private Medical Insurance. If you have an excess on your policy you are requested to pay this directly to M Betts.
Informed Consent for Face to face services during COVID-19 Public Health Crisis
This document contains important information about our decision (Client & Therapist) to carry out face to face (F2F) consultations during the public health pandemic COVD-19.
Decision to Meet F2F
You understand that by allowing a F2F contact with your therapist you are accepting the possibility of the risk of exposure to the Coronavirus. Due to the nature of F2F treatments we cannot comply to all social distancing strategies during the treatment period. ie. we are likely to be exposing client and therapist to contact with an individual <2m distance, for > 15 minutes. To minimise any risk of exposure the therapist will work in personal protective equipment (PPE); mask, gloves, aprons and in some instances visors. The client will be asked to wear a face mask issued by Melanie Betts Physiotherapy, to ensure suitability of the mask. There is a PPE surcharge of £2.00 per visit to go some way to meet PPE costs and room cleaning/airing time between clients.
Social distancing measures will be complied to as much as possible outside of the treatment rooms. We ask you to minimise time in waiting areas, not to bring partners to wait or attend with you unless previously discussed.
There will be a procedure on entering the clinic for hand hygiene and COVID symptom screening (temperature check/Oximetry reading) that you agree to participate with.
All our rooms and equipment will be sanitised thoroughly between clients in accordance with infection control protocols, and regularly through non treatment areas.
For home visits we will ask you to ventilate spaces where possible, complete hand hygiene and Covid screen as above. With therapist and client wearing PPE as above. All equipment used will be sanitised before and immediately after use.
To minimise exposure risk clients and therapist will
- Only keep appointments if feeling well, please inform us of need to cancel asap after developing any new symptoms including cough, temperature, loss of sense of taste or smell. If anyone in your family or close contact has also developed these symptoms in the last 48hrs we ask you to also cancel appointments until either you have had a negative COVID test or undergone the recommended period of isolation.
- Avoid touching the “T zone” (mouth, nose, eyes) during treatment
- If you test positive for COVID infection please inform us as early as possible. Will we also inform you if your therapist or a patient you may have followed into the clinic the same day, tests positive.
- You consent to be part of any contact tracing should anyone you come into contact with at the clinic test positive for COVID 19.
If at any time you feel unhappy with the risks involved in F2F appointments or become ill, you can at any time switch to virtual/remote consultation strategies.
Please remember Private medical insurance companies and PHE are all encouraging a virtual first approach. Your private medical insurers will cover virtual/remote physiotherapy.
